Required documentation before dispensing a Medicare Order

The Prescription

The prescription by the ordering physician must include Diagnosis, length of need & physician's name and signature.

Progress Notes

Progress notes from the Patient's Medical Record must be either one of the following statements:

• For Respiratory Dx: "Head needs to be elevated more than 30 degrees due to respiratory condition, wedge was considered inadequate."
• For Pain Dx: "Patient requires positioning of the body in ways not feasible with a regular bed to alleviate paid, wedge considered inadequate."

Nurse's written notes are accepted only if signed by the ordering physician.
